CERTIFIED NURSING ASSISTANT

JOB SUMMARY

  • Under the direction of a Professional Nurse, the Nursing Assistant provides basic care to patients, and demonstrates the skills, which qualify him/her to carry out specific routines and procedures for hospitalized patients.
  • The Nursing Assistant performs a variety of Patient Care functions and related non-professional services to assist nursing service personnel in the performance of their duties.

JOB RESPONSIBILITIES

  • Communicates problems to the nurse for each patient, in collaboration with each patient/family/significant other in order to address all identified needs.
  • Follows plan of care for each identified problem.
  • Reports needs of patient/family/significant other.
  • Practices quality nursing care in cooperation with the nurse.
  • Follows Nurse Manager's guidelines to reach established goals for patient care.
  • Demonstrates proper procedures in all aspects of care delivered.

  • Performs duties related to admission, discharge, and transportation of patients.

  • Take vital signs (BP, TPR, Weights). Reports all changes to the Nurse Manger/Nurse-in-Charge.

  • Provides personal care to patients (all aspects of Basic Nursing Care).
  • Reports on identified problems, care provided and patient's responses in order to meet patient and family needs to assure overall quality of care delivered.
  • Provides care to the geriatric patient and demonstrates awareness of special needs.
  • All other duties as assigned

QUALIFICATIONS

  • High School graduate preferred
  • Successful completion of NA certification program
  • Dependable and cooperative
  • Good communication skills
  • Have experience as a Nursing Assistant, or a graduate of a recognized Nursing Assistant Training Program.

CASE MANAGER
Job location: Boston, MA

JOB SUMMARY

Through knowledge of Utilization Review, Discharge Planning, Quality Improvement and Managed Care, the Case Manager evaluates patient's appropriateness for admission and continued stay, assesses their after-care needs, evaluates resources utilization, and promotes a quality, cost effective approach to meeting the goals of the hospital and post-hospital care plan.

JOB RESPONSIBILITIES

  • Collaborates with all disciplines involved in the pre-admission process to assess appropriateness of admission and meet payer admission requirements.
  • Collaborates with the Admissions Department to understand the criteria the patient meets for admission or assist in the decision-making.
  • Communicates with the insurer, as needed, for clarification of benefits and/or certification.
  • Incorporates a criterion-based system of chart review to determine appropriateness of continued stay and level of care.
  • Meets Federal, State and third party payer requirements for continued stay review.
  • Establishes sound communication lines with insurers to ensure clear expectations are set for concurrent medical record review.
  • Makes timely assessments and reassessments of the discharge planning needs of each individual patient, incorporating a multidisciplinary approach, involving the patient, family or significant others.
  • Evaluates 100% of admissions for discharge needs and documents the assessment within 2 business days of admission, in the integrated progress note section of the medical record.
  • Incorporates the Case Management Departments' Health Information documentation policy and procedure for discharge planning documentation.
  • Plans patient/family/team meetings, as needed, to establish the discharge planning process, estimated length of stay, treatment goals, disposition options and insurance guidelines.
  • Maintains a current knowledge of Federal, State and third party payer requirements for managing those patients not meeting admission/continues stay criteria.

QUALIFICATIONS

  • RN with a background in acute hospital care and 1 year minimum in Utilization Review, Discharge Planning or Managed Care.
  • Work requires specialized knowledge of pathphysiology, medical practice, and hospital service delivery in a chronic/rehabilitative setting.
  • Work requires an understanding of a PPS exempt health care facility, competency in the utilization review and discharge planning of med-psych, medical, and rehabilitative patient.
  • Work requires knowledge of federal, state, and third party payer regulations as they relate to utilization review, discharge planning and quality monitoring.
  • Work requires sensitivity to cultural diversity amongst patient and staff.

PHYSICAL THERAPIST/SUPERVISOR
Job location: Quincy, MA

Days + some weekends

Under the general direction of the Administrator at Radius Specialty Hospital - Quincy, the Physical Therapist should provide comprehensive physical therapy care to adults of all ages by evaluating dysfunction, identifying patient treatment goals, and establishing/implementing individualized treatment programs. Therapist is expected to provide education to the patient and family. Therapist is also responsible for working in a manner to prevent unsafe conditions and hazards in the work environment that may adversely affect the safety and health of self and others. 

QUALIFICATIONS:

  • Graduate of an AMA/APTA approved Physical Therapy program.
  •  Current Massachusetts state license (or eligible) to practice as a physical therapist.
  • Organizational and time management skills to: carry out responsibilities reliably and independently, complete tasks and meet deadlines in a satisfactory manner, delegate responsibilities effectively.
  •  Good interpersonal skills to interact with patients’ families and other health care professionals.
  • Analytical skills necessary to assess patients’ needs and develop treatment and discharge planning programs for patients on various/specific services.
  • Ability to lift and transfer heavy patients with established body mechanics techniques.
  • Ability to safely guard all patients during ambulation on level surfaces, inclined surfaces and stairs.
  • 2-3 years supervising a Rehab department
